Welcome to Restore & Rebuild Counseling Center. By using our website and services, you agree to the following terms. Please read them carefully.
Services
Restore & Rebuild Counseling Center provides professional counseling services, including individual and group therapy sessions via telehealth. All services are provided by licensed clinicians in accordance with applicable state laws and ethical guidelines.
Appointments & Cancellations
Sessions are scheduled by mutual agreement between the client and clinician. We require at least 24 hours' notice for cancellations. Late cancellations or missed sessions may be subject to a fee at the clinician's discretion.
Telehealth
Virtual sessions are available for clients residing in Louisiana (LA), Texas (TX), and Florida (FL). Telehealth appropriateness will be determined by the clinician. You are responsible for providing a private, confidential space for virtual sessions.
Payment
Fees for services are discussed and agreed upon before the first session. Payment is due at the time of service unless other arrangements have been made. We accept self-pay and certain insurance plans as communicated during intake.
Client Responsibilities
Clients are expected to provide accurate information, attend scheduled sessions, communicate openly with their clinician, and adhere to the policies outlined in the informed consent document provided at the start of services.
No Counselor-Client Relationship
Submitting an intake form or initial inquiry does not establish a counselor-client relationship. Such a relationship is only formed after a clinical assessment and mutual agreement to proceed with services.
Limitation of Liability
Restore & Rebuild Counseling Center and its clinicians shall not be liable for any damages arising from the use of this website or the services provided, to the extent permitted by applicable law.
Governing Law
These terms are governed by the laws of the State of Louisiana. Any disputes shall be resolved in the appropriate courts of Louisiana.
